If anyone finds themself in Paris and able to get more information (and photos?) of the leaves at the École nationale supérieure des beaux-arts de Paris, I would be grateful.

My warm thanks to François Avril for telling me about the leaves in Munich and Nuremburg, and for sending me images.

Edit, 23 Dec. 2016:
I have realised that all known leaves have 40 lines per column as far as the Ezra leaf in Paris, but thereafter they all have 38 lines. This means that the Paris leaf whose text is unidentified must belong to the first part of the book. I suspect that the initial "D" is the usual prologue before the start of Genesis "Desiderii mei" (Stegmuller no.285) on the recto, and and the Crucifixion miniature on the verso would have faced Genesis.

I have added the two Les Enluminures leaves to the list above.

Edit, 10 April 2017:
I have now found online images of the Paris leaves, and so I have been able to add page numbers above, and confirm that PC 22788-3 has the end of the General Prologue, the second normal prologue, and a miniature filling a space after the rubric that introduces Genesis. It shows a layman kneeling before the Crucifixion, holding up an open book, above which an angel holds a scroll "Liber est ante deum":
